Registration Instructions

- If you are sick, we recommend you delay vaccination until your symptoms have resolved.

- If you have a history of anaphylaxis to any of the ingredients of any of the COVID-19 vaccines (including polyethylene glycol and polysorbate), please consult with an allergist-immunologist to determine how you can most safely be vaccinated against COVID-19.

- NOTICE: The location of your second dose (if any) may be different than your first dose due to site closures and schedule changes. If the site of your first dose is not available, please choose a different location. If you need assistance, please call 510-268-2101.

- We will prioritize the COVID-19 AND flu vaccines to adults without insurance and adults whose insurance does not provide cost-free coverage. We also prioritize residents in the following priority ZIP Codes: 94601, 94603, 94605, 94606, 94607, 94621, 94541, 94544, 94545, 94577, 94578, 94580, 94560. - Getting a COVID-19 vaccine after you recover from COVID-19 infection provides added protection against COVID-19. You may consider delaying your vaccine by 3 months. CDC Frequently Asked Questions about COVID-19 Vaccination

- This service is free. Your health insurance company may help pay the cost of your immunization, but you will not be charged.
